Due to state regulations, lab visits in New York require an additional fee up to ~$248 per year for tests included with Function membership. This fee is charged by Quest Diagnostics during or after your lab visit.

You must accept Function's Terms of Service in order to use this membership. Purchase of this code enables a 1-year membership at Function Health. Your redemption code will be mailed to you. You must activate your membership with your redemption code at functionhealth.com, where you will be required to provide a credit card and accept automatic renewal terms. This annual membership will renew automatically at the full retail price after the initial term, and your payment method will be charged unless canceled. This purchase is not refundable.

Function is currently available only in the continental U.S. To access Function, you must have permanent residency, a valid U.S. address, and an active U.S. phone number. Lab visits are offered at 2,000+ locations across 48 states, but not yet in Rhode Island, Hawaii, or Puerto Rico. At this time, we do not offer access to testing for those who are pregnant. Must be 18 years and older.
